Banned filenames are not being recorded properly

As reported by Mike Oh:

When looking at the filename of banned files that are in the queue, what gets displayed is the following:

MIME-type (P=p003,L=1,M=multipa)

instead of the filename. When we look in the sql tables, this is what we see:

 mail_id    file_name   file_type
      39    MIME-type   P=p003,L=1,M=multipa
